Quest: The Way to an Otto's Heart.

Notes:

This otter mount is the reward for the quest, The Way to an Otto's Heart, but getting to that point is... kind of nuts.

First, obtain Aquatic Shades which have a 50% chance of appearing in an Immaculate Sac of Swog Treasures, purchased from The Great Swog in the Ohn'ahran Plains at 82.3, 73.2. You'll need a Gold Coin of the Isles to purchase a sac, and these are super-rare items that can be obtained through fishing, or can be traded up from lesser-quality coins - see the Wowhead link for more information.

Once you've obtained your Aquatic Shades, head to the Bubble Bath dive bar in the waters just north of the Obsidian Citadel, around 19.6 36.5 in The Waking Shores (the area above the bar is called the Hissing Grotto). By the bar, locate and stand on a small, 3x3 disco-style dance floor, wear your glasses and dance for 5 minutes. You'll know you have the right spot if you receive the debuff, Dance, Dance 'Til You're Dead which tracks your dancing time.

You'll know your dancing is complete when you are teleported. In your new location you'll see an Empty Fish Barrel, which you should pick up. You need to fill the fish barrel with 100 Frigid Floe Fish, 25 Calamitous Carp, and 1 Kingfin, the Wise Whiskerfish. These fish can be obtained by fishing around the Dragon Isles - see the Wowhead links for location info.

Once you've filled your barrel, head to the Hissing Grotto, around 20.3 39.7 and place your barrel on the ground. Otto should then appear with the quest, The Way to an Otto's Heart, granting you the mount.
